Information
Requests
At Practice Management, we try to do everything
we can to reduce unnecessary clerical work at
our client’s offices. On occasion, we may ask
for information that was missing from the
original paperwork. These are for claims that
will not be paid without the requested
information. By requesting the information up
front, we are able to eliminate claims that
would otherwise be denied or delayed. Insurance
carriers do not accept claims with missing
required fields.
The most frequently missing data elements are
the patient’s social security number and date of
birth. Patient’s insurance ID numbers are
frequently different than the patient SSN. All
claims require the patient’s SSN in addition to
the insurance ID. The SSN is now required even
for children.
